Information Governance
"Drowning in Data: The Driving Need for Information Governance"

The healthcare IT market is expected to grow at a compound annual growth rate of 7.4% for at least the next four years. Factors driving the growth of the healthcare information technology market are the rise in pressure to cut healthcare costs, growing demand to integrate healthcare systems, meaningful use, the rise in aging population, initiatives to reduce medication errors and improve patient safety and the rise in incidences of chronic disorders. This growth significantly increases the number of source systems originating healthcare data and the volume of patient data created. Patient data is expanding 50% per year. Currently we created 60 terabytes of information annually (1,000 times more than a decade ago).

This data is being exchanged through local, regional and state health information organizations and all of this exchange is dependent on the accurate and complete information from each Data Trading Partner.

The objectives of this session will review the current state of healthcare data exchange, the ONC's Patient Matching Initiative and the necessity of all provider organizations to develop and implement a strong information governance program to mitigate data exchange issues and improve EHR data integrity.

"The Hidden Challenges of Patient Portals"

Meaningful Use and patient engagement initiatives have created a strong need for provider organizations and HIOs to develop patient portals. There are a myriad of technical and policy challenges in building patient portals. The patient portal is a "release of information" function and all of the regulatory issues facing HIM when releasing patient records must be built into the security access capabilities as early adopters of these portals have learned. This session will focus on these issues and solutions to these challenges.

In recent years, the health information exchange (HIE) has emerged as a trusted way to share patient information across hospitals and health systems and improve the quality of patient care between organizations. However, a number of HIEs have implemented dangerous data integrity practices that may not only hinder data sharing, but threaten patient safety and care. This presentation will discuss the five most dangerous data integrity practices deployed by HIEs, how organizations can avoid these dangers and steps to overcome challenges to strong data integrity. It will also discuss data integrity best practices that will streamline the identification of duplicate or overlaid records and reduce the creation of new ones.

Health Information Integrity Summit

September 17, 2013
10:45 – 11:45 a.m.

Grant Landsbach, RHIA; Beth Just, MBA, RHIA

Lessons Learned from Strategic Alliances and the Quest for Data Integrity

Healthcare organizations are joining forces at an unprecedented rate. Data governance and data integrity face new challenges when organizations form alliances to better serve the healthcare community. Hear from experts and their first-hand experiences in facing and meeting the challenges of data governance and data stewardship as well as the downstream implications of the critical role in getting this right – the first time. Understanding the many moving yet connected parts that must be coordinated before, during and after site collaboration initiatives will help prepare HIM leaders for this rapidly growing trend.

Thursday, May 9th - 9:00am-10:00am

Speakers:
Beth Just, MBA, RHIA, FAHIMA - CEO/President Just Associates
Mary Anne Leach - CIO, Children’s Hospital Colorado

One of the greatest opportunities to impact the reduction of health care costs and improve quality care is integrating and sharing medical images. The Colorado Telehealth Network implemented the first state-wide, cloud-based image share via a vendor neutral archive (VNA) for medical images in the fall of 2012. This is a ground-breaking activity in health information exchange and was enabled due to the strong collaboration of CIOs across the state of Colorado.

HIM Involvement in Health Information Exchanges
Beth H. Just, MBA, RHIA
President, Just Associates, Inc.

Electronically linking disparate clinical data is challenging within a single healthcare organization; it is even more difficult for HIEs to achieve success exchanging data. Many HIEs start with the technology selection to be used to complete the data exchange without first defining their information exchange purposes or the business processes required to accomplish the data exchange. HIM Professionals need to be involved early in the HIE formation. We bring tremendous knowledge and skills that can help these HIEs be successful - and subsequently improve patient care for all of us. Tools have been developed to help bring a consistent message and support HIM professionals in this role. Learn about the deliverables of one of AHIMA's recent e-HIM workgroups, "HIM Principles in HIE" created to help HIM Professionals lead discussions on quality health information data exchange.

EHR success is severely compromised when physicians and other clinicians can't quickly and accurately locate their patients' online information. Data anomalies can create major frustrations for the clinician. Causes of data anomalies typically found in Healthcare databases will be highlighted in this presentation. Results of a study that examined the costs an organization incurs as a result of these data anomalies will be shared. Help your organization by learning strategies to identify and correct data anomalies and by identifying best practices for maintenance and prevention.

The EHR provides tremendous opportunity for improving both quality and efficiency of healthcare. And the research and other secondary data uses that can be attained will be enormous. However, EHRs are also creating unexpected and difficult challenges that can impede quality and efficiency improvements and that can actually cause harm. Electronic documentation challenges and their impacts on the legal health record will be discussed as will HIM's vital importance in the development of EHRs and the governance and policy development in their use. Several real-world examples will be discussed to heighten the audience's understanding of the subtleties of these issues.

Session Objectives:
• Demonstrate importance of data integrity and patient safety by walking through real stories of EHR mistakes leading to "near misses" in patient care
• Understand new HIM challenges that emerge as EHRs are implemented and utilized
• Discuss the evolving practice of HIM and emerging roles and opportunities for our profession - understand AHIMA's HIM "core model".

As with running hurdles, the development of a successful HIE is dependent upon developing a stride that allows the clearing of many hurdles - this stride development includes the technique (or technology), but also significant energy; those factors involving people and processes and a tenacious commitment to the fundament beliefs in what their organization is trying to achieve. Join us in learning about real world challenges in moving past writing and obtaining funding for an HIE, to actually achieving interoperability, financial sustainability and ultimately building a foundation that helps providers improve the patient care they deliver.
